Late Birth Certificate Application Process in Pakistan (2025)

You will need to apply and make a request for the late registration of birth; you will have to provide the following requirements:

Documents Needed:

  • Affidavit from parents or applicant
  • CNIC/B-form copies of parents
  • Hospital record of birth (if available)
  • School or vaccination record (if no hospital record)
  • Proof of residence (utility bill or lease)
  • NADRA Family Registration Certificate (FRC)
  • A written application explaining the cause of the delay

Where to Obtain a Birth Certificate in Pakistan?

The focal point for getting a manual or computerized birth certificate is your area Union Council office. In cities like Lahore, Karachi, Islamabad, Rawalpindi, Faisalabad, etc., computerized birth certificates are now issued through NADRA-linked Union Councils. For overseas Pakistanis, documents can be applied by initiating a process via Power of Attorney from overseas.

  • Sworn affidavit by both parents or the guardian
  • School admission record
  • Vaccination card
  • NADRA FRC that shows family relationship
  • Some Union Councils would also allow a witness or neighbor to attest to your birth particulars.

What If You’ve Lost The Original Birth Certificate?

If you previously had a birth certificate but lost the original, you can still apply for a duplicate copy by following these steps:

  • Go to the issuing Union Council.
  • Provide CNIC/B-Form and parents’ details.
  • Make an application for re-issuance of your birth certificate.
  • Wait for 3–5 days to obtain your certificate.
  • Certify by MOFA & Embassy (if required).
